Enrollment Phase SIS Administration
Once you have marketed SIS on your campus and a student is interested in SIS then proceed thru the following steps.
Step 1:
Have the student review the Online Pre-Service Overview, print out and complete reflection questionairre (which the member passes in with enrollment paperwork)
Step 2:
At this point, if the student is interested in enrolling in SIS then schedule and administer an SIS In-Person Orientation
What is an In-Person SIS Orientation?
Step 3:
Have member sign-in before the In-Person Orientation (keep a running file of SIS sign-in sheets on file for OIG auditors)
Step 4:
Collect Enrollment Paperwork (CP signs and dates the Member Agreement)
Step 5:
Make sure Criminal Record Check (either form below) is completed before enrolling member
NOTE: Beginning with all new enrollments starting August 1, 2010, the SIS Campus Coordinator (Campus Partner) must complete a check of the National Sex Offender Registry for "all" new members before they can be enrolled into the SIS program. The results must be placed in the member's file.

Step 6:
Invite and Enroll member into Egrants

Step 7:
Put together member file in standard member file format
Step 8:
Contact Site Supervisor

Enrollment Phase is complete! Move onto the Service Phase
